Medical and Dental Records
Your overall health impacts your oral health and, thus, affects what we need to do to deliver quality dental care that meets your needs. Proper dental care is particularly important to the prevention and management of systemic health conditions, like diabetes, cardiovascular disease, and complications arising from changes in hormone balances (e.g. pregnancy, menopause).
Please update your medical history form and dental history form if you have experienced recent changes in your health, including:
- Disease diagnosis
- Allergy development
- Change in medications
All patient forms should be updated and resubmitted if it has been one year or more since your last visit. You may also wish to review our Notice of Privacy Practices.
