Need to Bring With You
Before we can deliver excellent dental care, we need to gather background information and a signed consent form. If you would like to save yourself a few extra minutes, you may print out, complete, and bring with you the following paperwork:
- Patient Registration Form PDF format or Microsoft Word fillable format.
- Office Policy Form
- Dental History Form
- Medical History Form
- Acknowledgment of Receipt of Notice of Privacy Practices
(Please review our Notice of Privacy Practices before completing.) - For your convenience, you may also wish to print out a map of our location.
Please bring in a list of all medications and dosages (including natural supplements) that you take.
If you have dental insurance, please bring your insurance card. (We accept most insurance plans.)

What to Expect During Your First Visit
The relationship we seek to establish with each of our patients begins with a comprehensive exam, which includes:
- Detailed medical history
- Understanding your dental goals
- X-rays
- Periodontal assessment
- Occlusion check
- Clinical examination of teeth and soft tissues
During your exam, we discuss the importance of your dental health to your overall health and are happy to show you ways to improve your at-home dental hygiene to better maintain your smile and oral health.

If your dental insurance requires a co-payment or you will be paying out-of-pocket for your dental care, Fife Dental Care accepts:

- Most major credit cards
- Monthly payment plans through Care Credit

Our office is open Monday through Thursday from 8:00 am to 5:00 pm and Friday from 8:00 am to 12:00 pm (dental emergencies may be taken outside normal business hours).
